Basketball).<br \/>Vous recevrez alors un e-mail de la FFBB vous invitant \u00e0 compl\u00e9ter la demande de licence de l&rsquo;adh\u00e9rent\u00b7e.<\/p><p><b>\u27a2 <\/b><u>Si vous pensez ne pas avoir re\u00e7u cet e-mail, v\u00e9rifiez le dossier Spams\/Courrier ind\u00e9sirable de votre messagerie.<\/u><\/p><p>Dans cet e-mail, la FFBB vous fournira un lien \u00e0 cliquer pour acc\u00e9der constituer la demande de licence aupr\u00e8s de la FFBB.<\/p><p>Si vous \u00eates <u><b>majeur\u00b7e<\/b><\/u> et demandez une premi\u00e8re cr\u00e9ation de licence (voir cas ci-dessous), vous trouverez un lien &#8211; CERTIFICAT M\u00c9DICAL &#8211; dans l&rsquo;e-mail de la FFBB vous permettant de t\u00e9l\u00e9charger l&rsquo;imprim\u00e9 sp\u00e9cifique \u00e0 faire remplir par votre m\u00e9decin.<br \/>Si vous \u00eates <u><b>mineur\u00b7e<\/b><\/u>, quelque soit votre cas (renouvellement ou cr\u00e9ation de licence) aucun certificat m\u00e9dical n&rsquo;est \u00e0 fournir sous r\u00e9serve des r\u00e9ponses apport\u00e9es au questionnaire m\u00e9dical du formulaire e-licence.<\/p><p>Lors de la saisie des informations sur le &#8211; FORMULAIRE E-LICENCE -, plusieurs cas possibles :<\/p><p><b>\u27a2 L&rsquo;adh\u00e9rent\u00b7e a d\u00e9j\u00e0 \u00e9t\u00e9 licenci\u00e9\u00b7e \u00e0 la FFBB (renouvellement de licence)<\/b><\/p><p>Dans ce cas, l&rsquo;ensemble des informations d&rsquo;\u00e9tat civil et la photo d&rsquo;identit\u00e9 figurent d\u00e9j\u00e0 sur la demande de licence.<br \/>V\u00e9rifiez les informations pr\u00e9-enregistr\u00e9es (adresse, coordonn\u00e9es t\u00e9l\u00e9phoniques, etc.), corrigez-les si n\u00e9cessaire et validez la demande de licence.<br \/>Pour tous les adh\u00e9rent\u00b7e\u00b7s majeur\u00b7e\u00b7s qui renouvellent leur licence, vous aurez \u00e0 charger le recto\/verso de votre carte d&rsquo;identit\u00e9.<\/p><p><b>\u27a2 L&rsquo;adh\u00e9rent\u00b7e r\u00e9alise une premi\u00e8re demande de licence \u00e0 la FFBB (cr\u00e9ation de licence)<\/b><\/p><p>Dans ce cas, toutes les informations d&rsquo;\u00e9tat civil autres que le nom, pr\u00e9nom et adresse e-mail seront \u00e0 saisir sur la demande de licence et il faudra \u00e9galement charger une photo d&rsquo;identit\u00e9.
